Ontario County had 28,437 residents 65 and older in 2025, 25.1% of its population, the highest share of the five counties. The state licenses 7 assisted living buildings and 6 nursing homes there, most in Canandaigua and Geneva.
An older county with two hubs
Ontario County runs from Victor and Farmington, fast-growing suburbs at the Monroe County line, down to Canandaigua on its lake and the small city of Geneva on Seneca Lake. Its median age in 2025 Census estimates was 45.1, and its 65-and-older population grew from 24,183 in 2020 to 28,437. Licensed care is concentrated in two places: Canandaigua, the county seat, and Geneva, with Clifton Springs Hospital and Clinic's extended care unit in between.

Canandaigua
Canandaigua has the county's widest range: residences from under 20 beds to more than 70, one with Assisted Living Program beds for Medicaid residents, memory care beds on two campuses, and three nursing homes, including M.M. Ewing Continuing Care Center on the F.F. Thompson Hospital campus. The VA's Canandaigua medical center adds options for eligible veterans. Families here rarely need to leave town for assisted living, though they may for a specific kind of memory care.
Geneva and the eastern towns
Geneva's licensed assisted living is a single residence with memory care beds, in the town just outside the city, while nursing home capacity sits in the city near Geneva General Hospital. Families in Geneva often look east into Seneca County too, which is outside our directory but under the same New York rules. In the west, Victor has one small enhanced assisted living residence, and Victor and Farmington families commonly tour in Pittsford, Perinton and Canandaigua.
Victor and Farmington: growth at the Monroe line
Victor and Farmington have been among the fastest-growing towns in the region for two decades, and many of the older adults living there moved in as working families and are now aging in place in newer subdivisions. Licensed senior care has not kept pace inside those towns: the state roster shows one small enhanced assisted living residence in Victor and none in Farmington. Families there usually tour in Pittsford, Perinton and Canandaigua, all within a short drive along Route 96, Interstate 490 and Route 332.
Home care and adult day programs fill much of the gap. The Ontario County Office for the Aging can screen for EISEP aide services and home-delivered meals, and ElderONE's PACE program serves Ontario County for people who need long-term care and want to stay home.
For a family in Victor weighing two residences, the drive matters in both directions: a parent in Pittsford is close to Rochester hospitals and specialists, while a parent in Canandaigua is close to Thompson Hospital and the lake. Neither is wrong, but the choice decides which hospital system a parent is likely to use for years.
Hospitals and the continuum of care
Ontario County's three hospitals shape where people go after a stay. F.F. Thompson Hospital in Canandaigua is part of UR Medicine and has a continuing care center nearby, so many Canandaigua-area rehab stays happen close to home. Clifton Springs Hospital and Clinic, part of Rochester Regional Health, has an extended care unit licensed as a nursing home. Geneva General Hospital, part of Finger Lakes Health, sits next to the city's nursing home capacity.
For complex care, many Ontario County patients are transferred to Strong Memorial or Rochester General, which means a discharge may happen in Rochester even when the family lives in Canandaigua or Geneva. Ask the Rochester hospital's case manager for options back in Ontario County; there is no obligation to stay near the hospital for rehab or assisted living.
