Where does the directory's data come from?

Only from official bulk data: the Department of Health's facility roster and 2026 Q2 adult care capacity report on health.data.ny.gov, the nursing home bed census, NYS Health Profiles inspection and enforcement records, CMS's Provider Information dataset, and the Census Bureau's geocoder to place each address in its real town; we do not use aggregator listings, operator marketing or reviews.

Every listing page carries a “Source” line with the data date. Facility pages exist for every licensed residence with an inspection record and every CMS-certified nursing home; home care agencies and hospices appear as roster rows.

How do I use the listing pages?

Each listing page has a search box and filters for county, town and license type (and CMS rating for nursing homes), and every column can be sorted by clicking its heading, so you can, for example, list every residence in Greece by SNALR beds.

Why do the towns differ from mailing addresses?

The Rochester post office delivers to large parts of Greece, Irondequoit, Brighton, Gates and other towns, so we place each facility in its actual town or city using the Census Bureau's geocoder rather than by its mailing city.

A facility listed at a Rochester address may therefore appear on the Greece or Brighton page. The facility page shows both.
